REQUIREMENTS TO OPERATE UNDER GNSI SPONSORSHIP OF HOMES

The basic requirements to receive reimbursement for the CACFP are:
  • ​Must be registered Family Child Care Learning Home (FCCLH) and meet all of the required standards set by Bright from the Start
  • Serve meals that meet the nutritional standards established by USDA
  • Available and responsive to unannounced monitoring visits that will occur at least three times per year
  • Maintain all required forms, including but not limited to, Daily Attendance, Menus, Enrollment forms, and Income Eligibility Statements
  • Mail all required forms to GNSI by the 5th day of each month or submit online using our Minute Menu portal
  • FCCLH must remain compliant by GNSI and Bright from the Start standards
  • Follow all policies and procedures set forth by GNSI, Bright from the Start, and USDA
  • Promote the safety and well-being of the children in your care

HOW TO OPERATE EFFECTIVELY FOR REIMBURSEMENT

Below is a list of recordkeeping that must be maintained by FCCLH claiming reimbursement with GNSI
Daily recordkeeping requirements to receive reimbursement for the CACFP are:
  • Daily Menu for Children ages 1-12 years old
  • Infant Daily Menus
  • Point of Service Meal Count (Weekly Attendance and Meal Count Worksheet)
Monthly recordkeeping requirements to receive reimbursement for the CACFP are:
  • Monthly Record of Meals and Snacks Served
  • Copies of monthly claim for reimbursement
  • Original Posted Menus
Annual recordkeeping requirements to receive reimbursement for the CACFP are:
  • Roster
  • Racial Ethnic Data
  • WIC Information distributed to parents
  • Infant Affidavit on file
  • Enrollment Roster and Enrollment form for all children
  • Copies of past reviews
  • Current Registration/License
  • Affidavit to request to care for Additional Children
  • Agreement between sponsor and provider
  • Pre-operational Monitoring Visit Form
  • Enrollment Renewal Report
  • Proof of Program Eligibility (Tier I or Tier II)
  • School Zone Attendance Verification
  • Provider Income Eligibility Statement with Verification Materials
